INSTRUCTIONS FOR YOUR FIRST VISIT WITH DR. KISSEL

We would like to take this time to welcome you to our practice as well as inform you of our office policies.

• Your first appointment with Dr. Kissel will be approximately 45 minutes.
• A list of your current medications as well as any medical records you feel would be helpful during your consultation with Dr. Kissel.
• Please bring your current insurance cards and photo I.D.
• We are contracted with most major insurance companies, please come prepared to pay your co-pay at the time of service.
• Please complete the registration forms to the best of your ability and bring it with you to your appointment.
• We are happy to complete disability forms if you require surgery. There is a $10.00 charge to complete these forms with the exception of State Disability Forms and Medical Leave of Absence forms.
